Location: 3 days per week in McKeesport, 1 day in Homestead
Summary:
Co mmunity LIFE is an all-inclusive program that empowers older adults (age 55+) to remain at home, while preserving their dignity, independence, and quality of life. Our Physical Therapist works with Community LIFE participants in an effort to regain or compensate for mobility, strength, range of motion, pain, and/or discomfort with the expected outcome of helping improve the quality of life and independence. The Physical Therapist must demonstrate the knowledge and skills necessary to assess, plan care for and provide service to frail elders according to assigned responsibilities and Community LIFE standards. As the primary contact, the Physical Therapist performs utilization case management for all inpatient admissions and transfers to Skilled Nursing Facilities. The Physical Therapist directly supervises the PTA. The Physical Therapist will be required to make in-home visits as necessary for assessment and on-going case management.
Required Education & Experience:
Must be a graduate of a physical therapy curriculum approved by the American Physical Therapy Association (APTA) with the minimum of a Bachelor's degree and at least 2 years experience with the geriatric population.
Required Licensure/Certifications
- Valid PT license in the state of Pennsylvania.
- Valid PA Driver's license.

Compensation details: 35-45 Hourly Wage
